City of Hollywood Temporary Café Zone Registration Form

Any restaurant that wishes to establish outdoor dining areas in public or private, with the exception of the designated Hollywood Boulevard or Hollywood Beach Broadwalk Temporary Expanded Café Zones, must complete this registration form for the use of Temporary Café Zones and provide the following information:

Number of indoor seats previously (pre-emergency) at the restaurant
Number of outdoor seats previously (pre-emergency) at the restaurant (if not applicable, enter 0)
Days and hours that the new/additional outdoor seating will be used
Date new/additional outdoor seating is proposed to begin
Submit a drawing showing the layout and dimensions of the area to be used and the proposed location, size and number of tables, chairs, umbrellas, etc. Include the location of doorways, location of trees, parking meters, bus shelters, sidewalk benches, trash receptacles, light poles, and any other obstruction within the public right-of-way.
If a restaurant wishes to establish an outdoor dining area on private property, landlord/property owner authorization is required for new/additional outdoor seating. Submit an e-mail or signed letter from the landlord/property owner containing their contact information and approval.
Public Right-of-Way Authorization
If a restaurant wishes to establish an outdoor dining area on the public right-of-way in front of an adjacent business, property owner and tenant authorization is required. Submit e-mail(s) or signed letter(s) from the property owner and tenant, if applicable, containing their contact information and approval.
Vehicular Use Area Authorization
If the proposed new or additional outdoor seating includes parking spaces or other areas designed for or typically used by vehicles (“Vehicular Use Areas”), submit a detailed drawing showing: (i) the location of Vehicular Use Areas to be used for expanded outdoor dining; (ii) a safe path to and from one or more doors of the restaurant to the Vehicular Use Areas; (iii) any barrier or other method proposed to protect the Vehicular Use Areas from vehicular traffic. Additionally, if operation of the outdoor seating is proposed during other than daylight hours, state how lights or other methods will be used to ensure that drivers are aware of and can avoid the outdoor dining in the Vehicular Use Areas. Operations must comply with federal, state and local laws and ordinances.
Proof of Insurance
Use of the Temporary Café Zone program requires proof of insurance. Please upload a Certificate of Insurance which lists the City of Hollywood as an additional insured. The requirements for coverage are: • Not less than $1,000,000.00 per occurrence for bodily injury and property damage, $2,000,000 aggregate • Coverage should include Products and Personal Liability coverage.

Authorization To Begin Operation
An e-mail will be sent stating that the new/additional outdoor seating is approved to begin operation. City staff will subsequently visit the establishment to assist the operator in assuring compliance with all applicable rules. Please refer to the Back in Business Program Guide on the City website www.hollywoodfl.org for detailed information on Restaurant and Food Service Requirements or call the City of Hollywood Engineering Division at 954-921-3471.
