- Eligible applicants must be a United States Citizens or legal residents eligible to receive federal assistance.
- Eligible applicants shall not exceed CDBG program household income limits as certified by the City of Hollywood Community Development Division
- Eligible applicants are residents that are required to hook-up to the City Sewer system.
- Eligible applicants must own and occupy single-family homes as their principal residences and meet the following CDBG mandated household income limits adjusted for family size.
STEP 1: RESERVE CAPACITY CHARGES
The City provides a payment plan for Water and Sewer Reserve Capacity Charges. In order to receive assistance for Reserve Capacity Charges please contact Dianne Christie at DCHRISTIE@hollywoodfl.org or Gregory Rosenfield at GROSENFIELD@hollywoodfl.org. STEP 2: COMPLETE CDBG FUNDED SEWER CONNECTION APPLICATION
Complete CDBG Funded Sewer Connection Application (Click Here for Application) and Self-Certification Form (Click Here for Self Certification Form). The application requires attachments that confirm income eligibility. Attach all applicable documents as instructed on the application.
STEP 3: ATTACH THREE WRITTEN BIDS FOR ON-SITE WORK
The property owner must secure three written bids from a Licensed Contractor/Plumber in the City of Hollywood. In order to ensure that you are comparing bids appropriately it is mandatory to use the Bid Forms provided (Click Here for Bid Forms).
STEP 4: SIGN AND RETURN THE CDBG FUNDED SEWER CONNECTION APPLICATION WITH INCOME AND BID ATTACHMENTS
Mail completed CDBG Funded Sewer Connection Application with required attachments, Self-Certification Form, and Bid Forms to Anthony Grisby, City of Hollywood Community Development Division (Old Library Building) Hollywood, Fl. 33020 or scan and send the CDBG Funded Sewer Connection Application with required attachments and Bid Forms to email@example.com.
STEP 5: WAIT FOR APPROVAL – CONTRACTOR/PLUMBER IS NOT TO BEGIN WORK UNTIL STEPS 6 AND 7 ARE COMPLETED
Based on available funding, the Community Development Division will send a Sewer Connection Commitment Letter for the amount of CDBG funds to be applied to the user connection, a contract, and closing documents to complete Steps 6 and 7.
STEP 6: EXECUTE IN TRIPLICATE THE CONTRACT BETWEEN OWNER AND CONTRACTOR/PLUMBER FOR SEWER CONNECTION SERVICES
Execute three Contracts Between Owner and Contractor/Plumber for Sewer Connection Services. The Contract requires the signature of the Owner and the Contractor/Plumber in front of two witnesses. Return one executed contract to the City of Hollywood. A sample of the Contract is attached for your review. (Click Here for Sample Contract)
STEP 7: EXECUTE MORTGAGE AND NOTE TO SECURE CDBG INVESTMENT
CDBG funds shall be provided in the form of zero percent (0%) interest deferred payment “Forgiveness Loans”. The Forgiveness Loans are secured by a mortgage and promissory note that is forgiven after five (5) years of continued residency as the principal residence. Return executed mortgage and note to Anthony Grisby, City of Hollywood Community Development Division (Old Library Building) Hollywood, Fl. 33020.
STEP 8: INVOICE AND PAYMENT
The Contractor/Plumber Payments shall be made directly to Contractor/Plumber, on behalf of the Owner. After the connection is completed, the Contractor/Plumber and Owner shall complete an Application for Payment (Click Here for Application of Payment) Form with the attached Form W-9, Certificate of Completion, Termination of Notice of Commencement, Copies of all Closed Permits (Front and Back of Card), Final Release of Lien, Subcontractors Release of Lien (If applicable), and Contractor/Plumber Written Warranty Statement and submit to Anthony Grisby, City of Hollywood Community Development Division (Old Library) Hollywood, Fl. 33022. Contractor/Plumbers should allow approximately 30 days to receive payment.
