Contractor's Request for Clarification Form - This form is used when contractors have questions, need additional clarification on a project or design or would like to suggest a construction alternative. Forms may be submitted in person to the Building Department, City Hall, 3rd floor or via email.
Final Waiver of Lien Form - This form must be included in the final payment application package. It legally releases all liens arising from or filed in connection with the work.
Affidavit of Release of Liens - This form is included with the contractor's monthly payment application. The contractor shall state that all previous progress payments have been applied to discharge the contractor's obligations to its subcontractors and material suppliers in full.
